Therapy Specialists: A New Life HealthPRO-Heritage Company is seeking a Per Diem Physical Therapist with adult experience in Home Health, Skilled Nursing, ADHC, PACE, and/or Senior Living settings to provide in-person services on an as needed basis Monday through Friday between hours of 8:00 AM and 4:30 PM for a wonderful PACE [Program of All-Inclusive Care for the Elderly] Center in Modesto, CA! 


The Physical Therapist evaluates and treats participants through the use of a variety of therapeutic procedures designed to restore loss of muscle strength, coordination and body alignment, to relieve pain and to improve movement and function. The Physical Therapist participates as a member of the interdisciplinary team providing support and information within the area of Physical Therapy.


  • Develop and implement a program of physical therapy for patients enrolled in PACE and accessing day services that may include therapeutic exercise, neuromuscular reeducation, rehabilitation services, functional activities, gait training, orthotics training, prosthesis training, and massage. Modify the program as needed based on reevaluations and the patient population.
  • Perform assessments of patients to determine abilities.
  • Develop and implement an individualized plan of care to decrease or eliminate disability, increase or maintain capability for independence, and develop function to a maximum level.
  • Ensure that the individual plan of care is being followed by qualified support staff.
  • Provide therapies to include: muscle, nerve, joint and functional ability tests; treatment to relieve pain, develop or restore function; and assistance to achieve and maintain maximum performance using physical means such as exercise, massage, heat, sound, water, light or electricity.
  • Guide patients and caregivers in the use of adaptive devices and therapeutic, creative and self-care activities for improving function.
    Perform reassessments as indicated. Evaluate the effectiveness of treatment and modify goals as necessary. Traveling required as indicated.
  • Conducts home, hospital, and other relevant site visits as needed, and indicated by IDT and/or Therapy Supervisor.
  • Record and sign treatments in the patient's health record. Record, sign, and date quarterly progress notes. Review and initial all evaluations and the discharge summary.
  • Ensure services provided are in a therapeutic setting conducive to restoring dignity and self-esteem to all patients.
  • Ensure that required equipment and supplies for the physical therapy program are available or ordered as needed with the assistance of therapy supervisor.
  • Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government/regulatory agencies (PACE, ADHC, CMS, DHCS, etc) with the assistance of the therapy supervisor and compliance director.
  • Perform reassessments as indicated. Evaluate the effectiveness of treatment and modify goals as necessary.
  • Ensure services provided are in a therapeutic setting conducive to restoring dignity and self-esteem to all patients.
  • Direct oversight of Physical Therapy assistants.
    Participate in training programs for staff and caregivers.
  • Maintain and follow established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
  • Enhance professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.

  • Attend meetings as required and participate on committees as directed.
    Perform other related duties as assigned or requested.


  • Current California state licensure as a Physical Therapist
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support
  • Ability and means to travel as needed in a timely manner within this County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required
  • DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years;
  • Experience/Specialized skills (including Language)
  • 1-2 years of experience working with the older adult population preferred. New Grads are welcome to apply! 
  • Knowledge of appropriate and updated therapy equipment and technology to include aerobic and strength conditioning equipment tailored to the older adult population and use of mechanical assist devices
  • Ability to educate other staff on proper body mechanics
  • Ability to work effectively in a team environment
  • Demonstrated ability to communicate effectively and openly with other interdisciplinary team members on patient status and needs
  • Basic computer literacy to comply with department needs and expectations (i.e., E-HR documentation, obtaining background information and reports, following up on appointments, etc.)
