    The purpose of the Rehab Director Role is to anticipate, understand and fulfill the needs of the customers and partners we serve and promote an environment that ensures superior clinical and fiscal integrity. In order to achieve the vision of Heritage Healthcare, the Rehab Director serves as an advocate, educator and mentor to interdisciplinary team members. 

    Responsibilities

    CUSTOMER MANAGEMENT & MARKETING

    1. Represents HHI in a professional, courteous and competent manner as the point of contact for all facility/customer concerns or issues.
    2. Acts as a liaison between HHI management and facility administration, and is able to communicate assertively, diplomatically and clearly.
    3. Actively participates in trouble-shooting and problem resolution with Administrative staff of facility.
    4. Works closely with ED, Admin, DON and other key facility staff to identify new clinical programs, facility-specific clinical needs and on the development and integration of HHI programs within the facility’s marketing goals each year.
    5. Anticipates customer needs and helps to identify solutions for increased success.
    6. Actively involved in facility based committees and meetings , as needed, to help promote both facility goals and rehab goals and ensure the needs of all residents and customers are being satisfactorily met while remaining conscious of fiscally responsible time utilization.
    7. Develops formal mechanism to ensure open and timely communication with ED, Admin, DON and other key staff.
    8. Models the “Compassionate Care” Foundation not only with patients/families but with all levels of customers.
    9. Assists facility admissions coordinator in identifying appropriate resident referrals to the facility.
    10. Facilitates open communication with the facility business office manager to ensure prompt responses to Medicare claim reviews and denials and coordinates response to denials with AM, as needed.
    11. Participates in the implementation of the facility Quality Improvement Program to monitor program effectiveness and resident outcomes.
    12. Collaborate with appropriate facility staff on the purchasing process for supplies and equipment.
    13. Participates in/conducts patient care meetings as needed by the facility, to facilitate open and direct lines of communication regarding resident care.
    14. Leads efforts to educate the community, physicians, other healthcare providers as well as facility staff, residents and their families regarding rehab & facility services.
    15. Participates in Facility events to promote community awareness of our services and initiates facility events when a need is identified to educate the community, physicians, other healthcare providers, facility staff, residents and their families. 

    FISCAL AND CLINICAL RESPONSIBILITES

    1. Demonstrates strong clinical knowledge and understands the role of rehab within the (assigned) setting they work
    2. Plans/schedules/coordinates and directs resident rehabilitation services for the facility rehab department.
    3. Demonstrates good planning, organizational and interpersonal skills necessary to maintain a safe, orderly and goal-directed work environment for self and the team.
    4. Determines staffing needs and schedules to meet the demands of resident care and communicates staffing needs to AM and EM, assisting with recruiting as needed.
    5. Approves vacation requests and ensures sufficient coverage to meet the needs of every resident during staff vacation periods. Is proactive in anticipating and planning for staffing needs during Holidays.
    6. Independently analyze, interpret and achieve budget expectations by proactively ensuring productivity, RUG day distribution and clinical standards are accomplished on a consistent basis.
    7. Prepares/submits weekly/monthly reports timely and reflects a clear understanding of the outcomes being communicated.
    8. Ensures productivity standards for all staff and the rehab department as a whole - Communicates needs for adjustments and keeps all staff accountable to theses standards.
    9. Reviews financial performance and the components that affect it on a daily basis, making adjustments as needed, in order to ensure weekly/monthly targets are met.
    10. Understands and utilizes CareLink optimally to manage financial goals as well as provide the facility with necessary information to accurately complete the MDS and Billing.
    11. Coordinates the monthly billing process to ensure that information in CareLink is accurate and is submitted timely according to the established deadline to appropriate facility staff.
    12. Conducts weekly therapy staff meetings to ensure open and timely communication from both HHI and the facility and ensure collaborative approach to resident care and departmental functioning and coordinates participation of rehabilitation staff in department/facility meetings, as needed.
    13. Participates in open and direct communication with AM through both formal and informal channels.
    14. Ensures open and timely communication with all rehab staff and the patients/families and facility staff.
    15. Coordinates orientation and in-services/educational sessions for the facility/community.
    16. Works with MDS coordinator to effectively manage Med A caseloads both clinically and fiscally.
    17. Collaborates with HHI staff to identify and implement new or modified clinical programs for residents.
    18. Conducts/oversees completion of chart audits to ensure timely and accurate documentation on all residents and submits Quarterly Audits and findings to AM as scheduled. Communicates findings timely both to direct staff and AM.
    19. Develops and oversees a process for conducting screens and reviewing facility generated reports to ensure that all resident needs are being met and the customer’s skill mix goals are realized.
    20. Has a working knowledge of all three disciplines role within a LTC setting and is able to communicate how each discipline contributes to the care of the patients we serve.
    21. Oversees/monitors compliance with all company and government regulatory bodies as they pertain to the delivery of services provided by the rehab department.
    22. Oversees /coordinates clinical in-services including Clinical CareCapsules for the department and contacts available resources to ensure success.
    23. Assures ongoing compliance with Medicare/Medicaid/HIPAA/other regulatory bodies in the rehab department.
    24. Ensures rehab staff licenses and TB documentation are current and updated in a timely manner.
    25. Conducts annual performance reviews of rehab staff, including collaboration and consultation with AM and key facility staff and formal meeting with each staff member to discuss current performance and establish goals for future performance.
    26. Monitors the timeliness, professionalism and overall effectiveness of all tasks performed by the rehab team.
    27. Age-specific criteria: Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient’s status. Interprets the appropriate information needed to identify each patient’s requirements relative to his or her age specific needs and provides care according to departmental policies and procedures. 

    THE LEADERSHIP ROLE

    2. Establish open, direct lines of communication to ensure prompt and effective response to issues and inspire confidence in customers and staff.
    3. Able to communicate directly/assertively without personalizing including utilizing constructive strategies to resolve conflicts among staff/customers.
    4. Communicates in a manner which provides support, clarity and allows for sharing of ideas.
    5. Participates in mentoring, counseling and disciplinary action as needed with the guidance and involvement of the AM to aide and direct individual development of staff members and role model desired outcomes.
    6. Recognize the need to promote student programs and facilitate the staff’s willingness to work with students.
    7. Challenges rehab department staff both clinically and financially to increase staff’s operational awareness and professional development.
    8. Able to help develop specific behavioral/developmental strategies for all staff with the goal of enhancing the associate’s clinical skills.
    9. Recognizes opportunities for own personal growth and establishes goals and action plans in conjunction with the AM, to maximize potential.
    10. Encourage professional growth of therapy teams via mentoring and
    11. Helps to motivate staff to move out of comfort zone and challenge to grow clinically.
    12. Identifies opportunities for enhanced rehab program development and encourage staff to recognize program needs and develop programs, to improve resident care in the facility.
    13. Maintain and guide the implementation of all company and facility policies and procedures pertaining to rehabilitation department.
    14. Takes lead role in the orientation and ongoing nurturing and development of new staff to both HHI and the facility.
    15. Provides positive reinforcement to encourage team growth. Recognizes individual and team motivators and utilizes them directly and indirectly to acquire positive outcomes.
    16. Occasional overnight travel required.

    Qualifications

    1. Graduate of an approved program in the discipline of practice
    2. Valid professional license or eligibility for license in the state(s) where services are rendered.
    3. Two or more years of clinical experience in long-term care or sub-acute preferred. Demonstrates leadership ability in the healthcare field.
    4. Excellent communication and interpersonal skills.
    5. Knowledge of and ability to demonstrate skills related to management, planning, organization, budgeting and marketing. 6. Knowledge of local, state and federal regulations pertaining to facility and rehabilitation functions.
