Overview

HealthPro Heritage has a great Restorative Director Opportunity at Rockingham County Rehabilitation and Nursing Center in Brentwood, NH.

The Restorative Director is responsible for developing, implementing, and managing restorative nursing services that help residents achieve and maintain their highest practicable level of physical, mental, and psychosocial functioning in accordance with federal, state, and facility regulations.

 

This position could be filled with a LPN, RN, PT, OT, PTA, COTA

Responsibilities

  • Direct and oversee all aspects of the Restorative Nursing Program.
  • Develop individualized restorative nursing care plans in collaboration with the interdisciplinary team.
  • Identify residents who would benefit from restorative nursing interventions.
  • Supervise, mentor, and educate restorative nursing staff and nursing assistants.
  • Monitor resident progress and revise programs as needed to maximize functional outcomes.
  • Ensure compliance with CMS, state, and facility policies and regulations.
  • Maintain accurate documentation and program records.
  • Collaborate with Physical, Occupational, and Speech Therapy departments to support resident goals.
  • Conduct restorative nursing assessments and participate in care conferences.
  • Monitor quality indicators and implement performance improvement initiatives.
  • Educate residents and families regarding restorative nursing goals and interventions.
  • Assist with regulatory surveys and quality assurance activities.
  • Promote a culture of dignity, independence, and resident-centered care.

Qualifications

  • Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license, or PTA, or COTA licensure in good standing
  • Minimum of 2 years of long-term care experience.
  • Previous restorative nursing or rehabilitation experience preferred.
  • Supervisory or management experience preferred.
  • Knowledge of CMS regulations, MDS, restorative nursing documentation, and quality measures.
  • Excellent leadership, communication, and organizational skills.
  • Ability to work collaboratively with an interdisciplinary team.
  • Proficiency with electronic health records (EHR) systems.
