The School Social Worker (Medical Social Worker) provides clinical leadership and advocacy to ensure students and families receive the social and emotional support necessary for academic and personal success. By preparing detailed social histories and providing dedicated casework, the Social Worker bridges the gap between home, school, and community resources to supplement overall patient care.
Clinical Assessment: Conduct comprehensive social histories and assessments to identify areas where students require mental health counseling or social-emotional assistance.
Student Support: Facilitate remediation and social functioning by providing direct counseling and evidence-based treatment plans tailored to the school environment.
Multidisciplinary Collaboration: Actively participate in the Individualized Education Program (IEP) process and collaborate with teachers, administrators, and medical providers to support student goals.
Family Advocacy: Provide vital resources and guidance to families, aiding in the referral process to ensure a continuum of care and community support.
Case Management: Maintain accurate, confidential case files and progress reports to track student development and ensure compliance with agency standards.
Academic Integration: Evaluate student transcripts and academic records to identify social or environmental barriers to effective learning.
Communication: Serve as a primary liaison between the contracted agency, school staff, and parents to foster a collaborative and supportive educational atmosphere.
Professional Integrity: Stay current on research, evidence-based practices, and community resource availability to provide high-quality advocacy.
