Rehab Director – Sage Post Acute, Hayward, CA

HealthPro Heritage is seeking an experienced Rehab Director to lead therapy services at Sage Post Acute in Hayward, CA. This is a hands-on leadership role overseeing rehabilitation operations across Skilled Nursing (SNF), Outpatient, and Transitional Care, ensuring excellent patient outcomes and compliance with regulatory standards.
What You’ll Do:
Lead and manage the rehab team, including PT, OT, SLP, and support staff
Drive clinical excellence and operational efficiency across multiple therapy programs
Oversee PDPM compliance and quality outcomes
Collaborate closely with facility leadership to align therapy programs with organizational goals
