- Conduct health and fitness assessments for residents.
- Design exercise regiments, programs and goals for residents, taking into consideration level of function and fitness goals and using appropriate guidelines.
- Guide residents in proper exercise techniques.
- Instruct residents in use of fitness equipment and oversee the residents as needed while using the equipment, ensuring proper technique and appropriate level of difficulty.
- Design and instruct group sessions as appropriate to the resident population.
- Communicate improvements in fitness goals to residents to maintain interest and motivate residents to continue their programs.
- Provide staff and residents with educational sessions on fitness and wellness topics.
- Design and implement programs and incentives to increase awareness in health and fitness programs among the residents.
- Responsible for the routine maintenance of fitness equipment and reporting equipment malfunction and necessary replacement to ensure safety of residents.
- Collaborate with multiple disciplines and the therapy team to ensure the optimal exercise and fitness program, communicating any changes in the resident’s health status.
- Participate in team staff meetings as required by the facility and the company.
- Willingness to work with severely physically, emotionally and cognitively compromised patients.
- Willingness to work in environments with risk of personal injury, exposure to infections, waste materials, infectious diseases, and odors.
- Must be able to cope with the emotional stress associated with working with patients, families, and co-workers.
- Must be able to work independently with periodic (daily or less) support and directions.
- Basic knowledge of computers, typing abilities and willingness to utilize computers on a daily basis.
- Reports to work on time and coordinates schedule to achieve maximum productivity standards according to company guidelines.
- Ensures a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions, and OSHA standards for the healthcare professional.
- Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/appropriate personnel immediately.
- Responds to and acts appropriately in emergency or disaster situations.
- Adheres to high standards of cleanliness, grooming, hygiene and dress code.
- Provides accurate and timely documentation that meet facility and company guidelines.
- Ensures that progress is properly represented in documentation.
- Provides time reports and other necessary documentation to the company in a timely manner, in accordance with company policies.
- Adheres to all documentation requirements of Rehabilitation department, facility, and company.
- Maintains professional growth through a variety of sources (including study groups, in-services, seminars, online courses and self-study).
- Meets requirements in accordance with the regulations set forth by the appropriate professional organization to maintain any applicable certifications.
- Contributes to a positive work team by sharing information, using problem-solving methods, accepting new ideas, constructive feedback and advice from others.
- Demonstrates superior customer service through professionalism, courtesy, consistent and timely follow-up, and positive communication.
- Adheres to all policies of company, including job descriptions, certifications requirements, mission statement, HIPAA, confidentiality standards and patient rights.
