HealthPro Heritage is seeking a Full-Time Physical Therapist (PT) to join our therapy team in Roxboro, NC, within our Long Term Care division in a Skilled Nursing Facility (SNF) setting. This is a rewarding opportunity for a compassionate and skilled PT to make a meaningful impact on residents’ rehabilitation and quality of life.
Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
Provide hands-on physical therapy treatments to improve residents’ mobility, strength, and function
Collaborate with an interdisciplinary care team to deliver resident-centered care
Maintain accurate and timely documentation and effectively communicate resident progress
